Clinical Pharmacology Overview
Atorvastatin is an HMG-CoA reductase inhibitor (statin) that competitively inhibits the rate-limiting enzyme of cholesterol biosynthesis in the liver. Dosing in Adults
Low intensity: 10 mg once daily (reduces LDL by approximately 30%).
Moderate intensity: 10-20 mg once daily (reduces LDL by 30-50%). This is the most commonly used range in Indian practice.
High intensity: 40-80 mg once daily (reduces LDL by more than 50%). Reserved for ACS, established ASCVD, and familial hypercholesterolaemia.
Atorvastatin can be taken at any time of day (long half-life). Indian guidelines (ICMR, Cardiological Society of India) generally recommend starting at 10-20 mg and titrating based on lipid response.
Important Drug Interactions
- CYP3A4 inhibitors (clarithromycin, itraconazole, ketoconazole, HIV protease inhibitors, grapefruit juice): increase atorvastatin levels, raising myopathy risk
- Gemfibrozil: significantly increases statin myopathy risk (fenofibrate is preferred if combination needed)
- Cyclosporine: markedly increases atorvastatin levels -- use lowest dose
- Warfarin: may enhance anticoagulant effect; monitor INR
- Digoxin: atorvastatin may raise digoxin levels modestly

Side Effects and Monitoring
Common: headache, myalgia, GI discomfort, elevated transaminases.
Serious: myopathy (CK more than 10x ULN with symptoms), rhabdomyolysis (rare but potentially fatal), hepatotoxicity (rare), new-onset diabetes mellitus (dose-related, class effect).
Monitoring: baseline lipid profile, liver transaminases, CK (if symptomatic). Repeat lipid profile at 6-8 weeks to assess response. Annual LFTs. CK only if muscle symptoms develop.
Special Populations
Pregnancy: Category X. Absolutely contraindicated. Statins must be discontinued before conception and throughout pregnancy and lactation. Cholesterol is essential for foetal development.
Renal impairment: No dose adjustment needed as elimination is primarily hepatic/biliary.
Hepatic impairment: Contraindicated in active liver disease or unexplained persistent transaminase elevation above 3x ULN.
Elderly: No dose adjustment but start low. Higher myopathy risk due to reduced muscle mass and polypharmacy.
