Albendazole is the most widely used anthelmintic in India and is central to the national deworming programme. It is also indispensable for systemic parasitic infections including neurocysticercosis and hydatid disease, which are significant clinical problems in India.
Clinical Pharmacology Overview
Albendazole is a benzimidazole anthelmintic that binds to parasite beta-tubulin, inhibiting microtubule polymerisation. This disrupts glucose uptake, depletes glycogen stores, and impairs cell division in the worm, leading to its death. It has a broad spectrum covering nematodes, cestodes, and some trematodes.
Pharmacokinetics: Poorly absorbed orally (absorption increases 5-fold with fatty food). Rapidly metabolised in the liver to the active metabolite albendazole sulfoxide. Protein binding of sulfoxide is 70%. Half-life of albendazole sulfoxide is 8-12 hours. Excreted primarily in bile and urine.

Approved Indications in Indian Clinical Practice
- Intestinal nematodes: roundworm (Ascaris), hookworm (Ancylostoma, Necator), whipworm (Trichuris), pinworm (Enterobius)
- Strongyloidiasis
- Neurocysticercosis (Taenia solium larval stage in brain)
- Hydatid disease (Echinococcus granulosus) -- pre- and post-surgical or as primary treatment when surgery is not feasible
- Lymphatic filariasis (in combination with DEC or ivermectin in mass drug administration)
- Giardiasis (alternative agent)
- Cutaneous and visceral larva migrans
- National Deworming Day programme (mass administration)
Dosing in Adults
Routine deworming: 400 mg as a single oral dose.
Pinworm (Enterobius): 400 mg single dose, repeat after 2 weeks.
Strongyloidiasis: 400 mg twice daily for 3-7 days.
Neurocysticercosis: 15 mg/kg/day (max 800 mg/day) in 2 divided doses for 8-30 days. Take with fatty meals. Co-administer corticosteroids and antiepileptics.
Hydatid disease: 400 mg twice daily for 28-day cycles with 14-day drug-free intervals between cycles. Treatment duration: 3-6 months or longer.
Lymphatic filariasis (MDA): 400 mg single dose + DEC 6 mg/kg (or ivermectin 200 mcg/kg in areas with Loa loa co-endemicity).
Important Drug Interactions
- Dexamethasone: increases albendazole sulfoxide levels by approximately 50% (relevant in NCC treatment -- this is actually beneficial)
- Cimetidine: increases albendazole sulfoxide levels
- Praziquantel: increases albendazole sulfoxide levels (synergistic in NCC)
- Carbamazepine, phenytoin: may reduce albendazole sulfoxide levels (CYP inducers)
- Grapefruit juice: may increase absorption
- Theophylline: albendazole may increase theophylline levels
Side Effects and Monitoring
Single-dose deworming: generally very well tolerated. Mild abdominal pain, nausea, diarrhoea may occur due to worm expulsion.
Prolonged high-dose therapy (NCC, hydatid): hepatotoxicity (elevated transaminases in up to 16% of patients), leucopenia, pancytopenia, alopecia (reversible), headache, dizziness.
NCC-specific: worsening neurological symptoms due to inflammation from cyst degeneration (controlled with steroids), seizures, raised intracranial pressure.
Monitoring for prolonged courses: CBC and LFTs every 2 weeks. Pregnancy test before initiating prolonged therapy in women of childbearing age. Neuroimaging for NCC.
Special Populations
Pregnancy: Category C. Teratogenic in animal studies. Avoid in first trimester. WHO permits second/third trimester use in mass deworming programmes in high-prevalence settings. Women should not become pregnant during or within 1 month of treatment.
Renal impairment: No dose adjustment (primarily hepatic metabolism).
Hepatic impairment: Use with caution. Monitor LFTs closely. Avoid in severe hepatic disease.
Children: 400 mg single dose for children over 2 years. 200 mg for children 1-2 years (WHO mass deworming). Safety not established under 1 year.

Indian Regulatory Status
Albendazole is a Schedule H drug under the Drugs and Cosmetics Act. It is included in the NLEM of India and is one of the most affordable essential medicines. The Government of India conducts National Deworming Day (February and August) where albendazole 400 mg is distributed free to school-age children. It is manufactured domestically by numerous Indian pharmaceutical companies. Price is controlled under DPCO.
