What is INI-CET?
INI-CET (Institutes of National Importance Combined Entrance Test) is the PG entrance exam for AIIMS, JIPMER, PGIMER, NIMHANS and SCTIMST — the most prestigious postgraduate medical training programmes in India. Conducted by AIIMS New Delhi, it offers admission to MD, MS, DM, MCh and MDS programmes at these autonomous institutes.
Unlike NEET-PG, which is managed by NBEMS, INI-CET is conducted independently and typically held twice a year (May and November). The two exams test the same MBBS syllabus but differ significantly in question style.
INI-CET questions are generally more clinical, image-based and application-oriented than NEET-PG. The exam rewards candidates who can integrate knowledge across subjects and apply it to clinical scenarios — making systematic MCQ practice and clinical case exposure essential.
INI-CET vs NEET PG: Key Differences
Both exams test the MBBS syllabus, but the approach is different:
Question style: INI-CET is more clinical and image-based. Expect more questions with X-rays, histopathology slides, ECGs and clinical photographs. NEET-PG has shifted toward clinical scenarios but still includes more recall-based questions.
Marking: INI-CET has 200 questions in 3 hours with +4/-1 marking (800 total marks). NEET-PG has 180 questions in 3.5 hours with +4/-1 marking (720 total marks). INI-CET gives you less time per question.
Difficulty: INI-CET is generally considered harder in terms of question quality. The pass rate is lower and the competition is for fewer seats at more prestigious institutions.

INI-CET Exam Pattern 2026
Format: 200 single-best-answer MCQs. Computer-based test. Duration: 3 hours (180 minutes). That gives you 54 seconds per question — significantly faster than NEET-PG.
Marking: +4 for correct, -1 for incorrect, 0 for unanswered. Total marks: 800. Negative marking applies, so blind guessing is penalised.
Subjects: All 19 MBBS subjects with a higher proportion of image-based and clinical questions compared to NEET-PG.
