Melanoma
Apply the ABCDE rule: Asymmetry, Border irregularity, Colour variation, Diameter over 6 mm and Evolution over time. Any changing pigmented lesion, especially with these features, needs urgent excision biopsy rather than reassurance.
Psoriasis
Well-demarcated erythematous plaques with a silvery scale, typically on extensor surfaces and the scalp. Removing the scale reveals pinpoint bleeding (the Auspitz sign). Look for nail pitting and ask about joint symptoms.
