Drug action
Teprotumumab is a recombinant human monoclonal antibody that binds to and blocks insulin-like growth factor-1 receptor (IGF-1R) activation.
Indications and dose
**Thyroid eye disease (under expert supervision)**
- **Adult** (By Intravenous Infusion): Initially 10 mg/kg for 1 dose, followed by 20 mg/kg every 3 weeks for 7 doses.
Cautions
Diabetes mellitus or hyperglycaemia; hearing impairment; inflammatory bowel disease (risk of exacerbation-consider discontinuation); infusion-related reactions Cautions, further information Hearing impairment Hearing impairment, including hearing loss, may occur and be permanent in some cases. The benefit versus risk for patients with pre-existing hearing impairment, or those who experience severe hearing impairment during treatment, should be assessed. Auditory function should be monitored before, during, and after treatment. Patients should be advised to stop smoking, avoid prolonged loud noises, and seek medical attention if they experience any signs of altered hearing. M Infusion-related reactions Infusion-related reactions can occur and teprotumumab should only be administered when appropriately trained staff are available; these reactions can be managed by interrupting or stopping the infusion and appropriate management initiated. Patients should be closely monitored for signs of infusion-related reactions during and for 90 minutes after completion of the infusion. Consider premedication with an antihistamine, antipyretic, and corticosteroid, and reducing the rate of subsequent infusions. M

Common or very common Alopecia; autophony; COVID-19; diarrhoea; dry skin; ear discomfort; ear disorders; fatigue; headache; hyperglycaemia; infusion related reaction; menstrual cycle irregularities; muscle spasms; nail discolouration; nail disorders; nausea; taste altered; tinnitus; weight decreased Uncommon Diabetic ketoacidosis; hyperacusia; inflammatory bowel disease aggravated; tympanic membrane disorder Frequency not known Hearing loss (may be permanent); hyperglycaemic hyperosmolar nonketotic syndrome
Pregnancy
Avoid (toxicity in animal studies). M
Breast feeding
Specialist sources indicate use with caution, especially if breast-feeding a neonate or preterm infant (no information available). Large molecular weight suggests limited excretion into milk and drug molecule likely to be partially destroyed in the infant's gastro-intestinal tract; waiting for at least 2 weeks postpartum to resume treatment may minimise transfer to infant.
