Sotatercept is an activin signalling inhibitor which prevents the thickening and narrowing of small blood vessels in the lungs.
Indications and dose
**Pulmonary arterial hypertension (under expert supervision)**
- **Adult** (By Subcutaneous Injection): Initially 0.3 mg/kg for 1 dose (max. per dose 55 mg), for dose adjustments or interruption due to increase in haemoglobin concentration or decrease in platelet count-consult product literature, then maintenance 0.7 mg/kg every 3 weeks (max. per dose 120 mg), maintenance dosing to be started 3 weeks after the initial dose, for dose adjustments or interruption due to increase in haemoglobin concentration or decrease in platelet count-consult product literature.
Cautions
Polycythaemia
Contraindications
Platelet count less than 50 x 10 9 cells/litre (do not initiate)
Side effects
Common or very common Diarrhoea; dizziness; haemorrhage; headache; hypertension; skin reactions; telangiectasia; thrombocytopenia Frequency not known Intracranial haemorrhage; polycythaemia
Pregnancy
Avoid (toxicity in animal studies). M
Breast feeding
Specialist sources indicate use with caution (no information available). Large molecular weight suggests limited excretion into milk and drug molecule likely to be partially destroyed in the infant's gastro-intestinal tract.
