Clinical overview
Salbutamol is the most important bronchodilator in clinical medicine and is the prototype short-acting beta-2 agonist (SABA). It is the first-line reliever medication for acute bronchospasm in asthma, COPD exacerbations, and exercise-induced bronchoconstriction. Its rapid onset of action (within 5 minutes by inhalation) and short duration (4-6 hours) make it ideal as a rescue inhaler. According to GINA (Global Initiative for Asthma) guidelines, salbutamol should be used on an as-needed basis for symptom relief; overuse (>2 canisters per month or >12 puffs per day) indicates poorly controlled asthma and the need for step-up of controller therapy (inhaled corticosteroids). In India, salbutamol is one of the most commonly prescribed respiratory medications, available in metered-dose inhalers (MDI), nebuliser solutions, oral tablets, and syrups. The inhaled route is strongly preferred over oral formulations because inhaled salbutamol delivers drug directly to the airways with minimal systemic absorption, while oral salbutamol causes more systemic side effects (tremor, tachycardia, hypokalaemia) with less bronchodilation. Salbutamol is also used in obstetric practice as a tocolytic to delay preterm labour (by relaxing uterine smooth muscle via beta-2 receptors), though its use for this indication has declined in favour of nifedipine and atosiban due to maternal cardiovascular side effects.
Pharmacological class
Salbutamol (Albuterol) belongs to the Short-acting beta-2 adrenergic agonist (SABA; bronchodilator) class. Salbutamol selectively stimulates beta-2 adrenergic receptors on airway smooth muscle cells. Receptor activation stimulates adenylyl cyclase via Gs protein, increasing intracellular cAMP, which activates protein kinase A (PKA). PKA phosphorylates multiple targets leading to smooth muscle relaxation: inhibition of myosin light chain kinase, opening of large-conductance calcium-activated potassium channels (hyperpolarisation), and reduction of intracellular calcium. The net effect is bronchodilation within 5 minutes of inhalation.

Regulatory status
Salbutamol (Albuterol) is classified under Schedule H in India under the Drugs and Cosmetics Act, 1940. Schedule H prescription drug. Metered-dose inhalers (MDIs) and nebuliser solutions are available. Oral formulations (tablets, syrups) are also Schedule H but are discouraged due to inferior efficacy-to-side-effect ratio.
Indications
- Acute bronchospasm in asthma (rescue/reliever therapy)
- COPD exacerbations (acute bronchodilation)
- Exercise-induced bronchoconstriction (prophylaxis -- 2 puffs 15-20 min before exercise)
- Acute severe asthma and status asthmaticus (nebulised, high-dose)

- Hyperkalaemia (nebulised -- drives K+ intracellularly, same mechanism as in beta-2-mediated glycogenolysis)
Dosing
MDI (acute relief): 100-200 mcg (1-2 puffs) as needed, up to 4-6 times daily. Use with a spacer for optimal delivery. Nebulised (acute severe asthma): 2.5-5 mg every 20 minutes for 3 doses, then every 1-4 hours. Oral: 2-4 mg TDS-QDS (inferior to inhaled; avoid if possible). IV (status asthmaticus): 5 mcg/min infusion, titrate. Tocolysis (IV): 10 mcg/min, increase every 10 min. Exercise prophylaxis: 2 puffs 15-20 minutes before exercise.
Contraindications
- Known hypersensitivity to salbutamol
- Tachyarrhythmias (beta-2 stimulation increases heart rate)
- Hypertrophic obstructive cardiomyopathy (HOCM)
- Unstable angina (relative -- use with caution)
Adverse effects
- Tremor (most common, due to beta-2 receptor stimulation in skeletal muscle)
- Tachycardia and palpitations (beta-1 cross-reactivity at high doses)
- Hypokalaemia (beta-2 stimulation drives K+ intracellularly -- important in acute asthma with repeated dosing)
- Headache
- Muscle cramps
- Paradoxical bronchospasm (rare, due to propellants in MDI)
- Lactic acidosis (rare, with high-dose IV or nebulised therapy in severe asthma)
- Hyperglycaemia (glycogenolysis via beta-2 receptors)
Drug interactions
- Beta-blockers (propranolol) -- antagonise bronchodilator effect; AVOID non-selective beta-blockers in asthma (life-threatening bronchospasm)
- Theophylline -- additive bronchodilation but also additive tachycardia and hypokalaemia
- Corticosteroids and diuretics -- additive hypokalaemia
- Digoxin -- hypokalaemia from salbutamol increases digoxin toxicity risk
- MAO inhibitors and TCAs -- may potentiate cardiovascular effects
Pregnancy and lactation
Category C. Widely used in pregnancy for asthma (uncontrolled asthma is far more dangerous to the fetus than salbutamol). GINA guidelines recommend that pregnant women should not discontinue asthma medications. Also used as a tocolytic for preterm labour (IV), though maternal side effects (pulmonary oedema, tachycardia) limit this use. No established teratogenicity.

A 12-year-old boy with known asthma presents with acute severe breathlessness, widespread wheeze, and SpO2 88% on room air. He has used his salbutamol inhaler 6 times in the past 2 hours with minimal relief. What is the emergency management? Answer: This is acute severe asthma. Immediate management: (1) High-flow oxygen to maintain SpO2 94-98%, (2) Continuous nebulised salbutamol 5 mg every 20 minutes for 3 doses (driven by oxygen, not air), (3) Add ipratropium bromide 0.5 mg to the first 3 nebulisations, (4) IV/oral corticosteroids (prednisolone 1-2 mg/kg or hydrocortisone 4 mg/kg IV), (5) If no improvement: IV magnesium sulphate 40 mg/kg over 20 minutes, consider IV salbutamol infusion. Why are non-selective beta-blockers contraindicated in asthma?
Non-selective beta-blockers (propranolol, carvedilol, labetalol) block beta-2 receptors on bronchial smooth muscle, preventing the bronchodilatory effect of circulating adrenaline and administered salbutamol. This can precipitate life-threatening bronchospasm in asthmatic patients. Even beta-1 selective blockers (atenolol, metoprolol, bisoprolol) lose selectivity at higher doses and should be used with caution. If a beta-blocker is needed in an asthmatic patient, use a cardioselective agent at the lowest effective dose.

This is the same mechanism exploited when nebulised salbutamol is used to treat hyperkalaemia in emergency settings.
