Indications and dose
**Distal renal tubular acidosis**
- **Adult** (By Mouth): Initially 1 mEq/kg daily in 2 divided doses, adjusted in steps of 0.5 mEq/kg daily if required, to be adjusted until optimal response is achieved, use the lowest maximum daily dose; maximum 10 mEq/kg per day; maximum 336 mEq per day.
**Distal renal tubular acidosis for potassium citrate with potassium bicarbonate**
- **Child 1-3 years** (By mouth): Initially 4 mEq/kg daily in 2 divided doses, adjusted in steps of 1.5 mEq/kg daily if required, to be adjusted until optimal response is achieved, use the lowest maximum daily dose; maximum 10 mEq/kg per day; maximum 336 mEq per day.
- **Child 4-11 years** (By mouth): Initially 2 mEq/kg daily in 2 divided doses, adjusted in steps of 1.5 mEq/kg daily if required, to be adjusted until optimal response is achieved, use the lowest maximum daily dose; maximum 10 mEq/kg per day; maximum 336 mEq per day.
- **Child 12-17 years** (By mouth): Initially 1 mEq/kg daily in 2 divided doses, adjusted in steps of 1 mEq/kg daily if required, to be adjusted until optimal response is achieved, use the lowest maximum daily dose; maximum 10 mEq/kg per day; maximum 336 mEq per day.
Cautions
For potassium citrate with potassium bicarbonate Conditions predisposing to hyperkalaemia; gastrointestinal disorders Cautions, further information Gastrointestinal disorders Gastrointestinal disorders such as malabsorption, delayed gastric emptying, diarrhoea, nausea and vomiting could affect efficacy and safety. Monitor plasma bicarbonate levels regularly and adjust dose as clinically indicated. M
Contraindications
For potassium citrate with potassium bicarbonate Hyperkalaemia
Side effects
For potassium citrate with potassium bicarbonate Common or very common Diarrhoea; gastrointestinal discomfort; gastrointestinal disorder; nausea; vomiting
Pregnancy
For potassium citrate with potassium bicarbonate Use only if potential benefit outweighs risk-increased risk of developing hyperkalaemia. M
Breast feeding
For potassium citrate with potassium bicarbonate Present in milk but not known to be harmful. M
Renal impairment
For potassium citrate with potassium bicarbonate See Prescribing in renal impairment . Avoid if eGFR is less than 45 mL/min/1.73m 2 . M Avoid if eGFR is between 45 and 59 mL/min/1.73m 2 and plasma potassium levels are elevated. M Caution if eGFR is between 45 and 59 mL/min/1.73m 2 -monitor renal function and plasma potassium levels prior to and after initiation of treatment or dose increase, then at least twice yearly thereafter. M
