Indications and dose
**Treatment of uncomplicated falciparum malaria [using 320 mg/40 mg tablets],Treatment of uncomplicated non-falciparum malaria [using 320 mg/40 mg tablets]**
- **Adult** (By Mouth): (body-weight 75 kg and above) 4 tablets once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.

- **Child 6 months-17 years (body-weight 5-7 kg)** (By mouth): 0.5 tablet once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
- **Child 6 months-17 years (body-weight 8-10 kg)** (By mouth): 0.75 tablet once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
- **Child 6 months-17 years (body-weight 11-16 kg)** (By mouth): 1 tablet once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
- **Child 6 months-17 years (body-weight 17-24 kg)** (By mouth): 1.5 tablets once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
- **Child 6 months-17 years (body-weight 25-35 kg)** (By mouth): 2 tablets once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
- **Child 6 months-17 years (body-weight 36-74 kg)** (By mouth): 3 tablets once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
- **Child 6 months-17 years (body-weight 75 kg and above)** (By mouth): 4 tablets once daily for 3 days, max. 2 courses in 12 months; second course given at least 2 months after first course.
Contraindications
For piperaquine phosphate with artenimol Bradycardia; congenital long QT syndrome; electrolyte disturbances (particularly hypokalaemia, hypocalcaemia, or hypomagnesaemia); family history of sudden death; heart failure with reduced left ventricular ejection fraction; history of symptomatic arrhythmias; left ventricular hypertrophy; risk factors for QT c interval prolongation; severe hypertension
Side effects
For piperaquine phosphate with artenimol Common or very common Anaemia; arrhythmias; asthenia; fever; headache; QT interval prolongation Uncommon Abdominal pain; appetite decreased; arthralgia; cardiac conduction disorder; cough; diarrhoea; dizziness; hepatic disorders; increased risk of infection; myalgia; nausea; pruritus; seizure; vomiting Frequency not known Eosinophilia; haemolytic anaemia Side-effects, further information Haemolytic anaemia and autoimmune haemolytic anaemia can occur up to one month after starting treatment.
Pregnancy
For piperaquine phosphate with artenimol Avoid in first trimester (teratogenic in animal studies); may be used in second and third trimester when other artemisinin combination therapies are unsuitable (large amount of data indicates no fetotoxicity); M recommendation also supported by specialist sources.
Breast feeding
For piperaquine phosphate with artenimol Specialist sources indicate present in milk but not known to be harmful.
Hepatic impairment
For piperaquine phosphate with artenimol Caution in jaundice or in moderate to severe impairment (no information available)-monitor ECG and plasma potassium concentration. M
