Drug action
Orphenadrine exerts its antiparkinsonian action by reducing the effects of the relative central cholinergic excess that occurs as a result of dopamine deficiency.
Indications and dose
**Parkinsonism,Drug-induced extrapyramidal symptoms (but not tardive dyskinesia)**
- **Adult** (By Mouth): Initially 150 mg daily in divided doses, then increased in steps of 50 mg every 2-3 days, adjusted according to response; usual dose 150-300 mg daily in divided doses; maximum 400 mg per day.
- **Elderly** (By Mouth): Preferably dose at lower end of range.
Cautions
Cardiovascular disease; elderly; hypertension; in patients susceptible to angle-closure glaucoma; liable to abuse; prostatic hypertrophy; psychotic disorders; pyrexia
Contraindications
Acute porphyrias ; gastro-intestinal obstruction
Side effects
Common or very common Accommodation disorder; anxiety; dizziness; dry mouth; gastrointestinal disorder; nausea Uncommon Confusion; constipation; coordination abnormal; euphoric mood; hallucination; insomnia; sedation; seizure; tachycardia; urinary retention Rare or very rare Memory loss
