Indications and dose
**Benign gastric, duodenal or NSAID-associated ulceration**
- **Adult** (By Mouth): 300 mg once daily for 4-8 weeks, dose to be taken in the evening, alternatively 150 mg twice daily for 4-8 weeks; maintenance 150 mg once daily, dose to be taken at night.
**Gastro-oesophageal reflux disease**
- **Adult** (By Mouth): 150-300 mg twice daily for up to 12 weeks.
Cautions
For all h2-receptor antagonists Signs and symptoms of gastric cancer Cautions, further information Gastric cancer H 2 -receptor antagonists might mask symptoms of gastric cancer; particular care is required in patients presenting with 'alarm features' in such cases gastric malignancy should be ruled out before treatment.
Side effects
For all h2-receptor antagonists Common or very common Diarrhoea; dizziness; fatigue; headache; myalgia; skin reactions Uncommon Confusion; depression; erectile dysfunction; gynaecomastia; hallucination; hepatitis; leucopenia; nausea Rare or very rare Agranulocytosis; alopecia; arthralgia; atrioventricular block; fever; pancytopenia; thrombocytopenia Side-effects For nizatidine Rare or very rare Anaemia; hepatic disorders; hyperuricaemia; serum sickness; vasculitis Frequency not known Hyperhidrosis
Interactions
**Severe interactions:**
- Absorption of nizatidine is not clinically significantly affected by food intake, anticholinergic agents, or antacids.
**Other interactions (5):**
- There is evidence that oral nizatidine does not affect the serum levels of concomitantly-administered aminophylline, theophylline, chlordiazepoxide, diazepam, lidocaine, phenytoin, ibuprofen,...
- Nizatidine does not inhibit the hepatic cytochrome P450-linked drug metabolising enzyme system, but may increase absorption of salicylates when they are used in very high dosage.
- However, nizatidine and other histamine H 2 -receptor antagonists can reduce the gastric absorption of drugs whose absorption is dependent on an acidic gastric pH.
- Approximately 35% of nizatidine is bound to plasma protein.
- Warfarin, diazepam, paracetamol, propantheline, phenobarbitone, and propranolol did not affect plasma protein binding of nizatidine in vitro .
Pregnancy
Manufacturer advises avoid unless essential.
Breast feeding
Amount too small to be harmful.
Hepatic impairment
Manufacturer advises caution.
Renal impairment
Use with caution. M Dose adjustments Use half normal dose if creatinine clearance 20-50 mL/minute. M Use one-quarter normal dose if creatinine clearance less than 20 mL/minute. M See Prescribing in renal impairment .
