Indications and dose
**Emergency treatment of life-threatening bradycardia [when treatment with atropine is ineffective] (under expert supervision)**
- **Adult** (By Intravenous Infusion): Initially 5 micrograms/minute, adjust dose according to response. Consult Resuscitation Council (UK) guidance for further information.
**Severe or permanent bradycardia (under expert supervision),Stokes-Adams syndrome (under expert supervision)**
- **Adult** (By Intravenous Infusion): Carefully titrate dose according to heart rate and response-consult local protocols or product literature.
Cautions
Cardiovascular disorders e.g. chronic coronary insufficiency, arrhythmias and hypertension; controlled hyperthyroidism; convulsive disorders; diabetes mellitus; hypovolaemia
Contraindications
Acute coronary insufficiency, especially acute myocardial infarction (except in complete atrioventricular block with extreme bradycardia); angina pectoris; digitalis intoxication; pre-existing ventricular arrhythmias; tachycardia (heart rate >130 beats per minute); uncontrolled hyperthyroidism
Side effects
Frequency not known Angina pectoris; arrhythmias; headache; hot flush; hypotension; nausea; tremor
Interactions
**Severe interactions:**
- Doxapram and MAOIs may cause a risk of severe hypertension.
**Other interactions (7):**
- Isoprenaline should not be used during anaesthesia with chloroform, cyclopropane, halothane, or other halogenated anaesthetic agents because they may cause or worsen ventricular arrhythmia.
- Isoprenaline should not be administered simultaneously with MAOIs.
- Isoprenaline toxicity increases when administered at the same time as other cardiotonics or drugs stimulating the central nervous system (such as sympathomimetics, theophylline or thyroid hormone...
- Isoprenaline may worsen cardiovascular side effects of tricyclic antidepressants such as imipramine.
- Administration of entacapone may enhance the effect of isoprenaline.
- Isoprenaline Hydrochloride Concentrate for Solution for Infusion may increase the risk of ergotism, if given together with ergotamine.
Pregnancy
Avoid unless potential benefit outweighs risk. M
Breast feeding
Specialist sources indicate probably compatible-careful monitoring of infant recommended. Risk is low if infant is breastfed at least half an hour after administration of isoprenaline.
