Drug action
Iptacopan is a complement inhibitor which targets factor B and prevents the activation of C3 convertase, thereby inhibiting the complement cascade that leads to haemolysis.
Indications and dose
**Paroxysmal nocturnal haemoglobinuria (under expert supervision)**
- **Adult** (By Mouth): 200 mg twice daily.
Contraindications
Patients unvaccinated against Neisseria meningitidis , and Streptococcus pneumoniae ; unresolved infection caused by encapsulated bacteria (including Neisseria meningitidis , Streptococcus pneumoniae , and Haemophilus influenzae type b) Contra-indications, further information Vaccination against serious infection To reduce the risk of serious infection, it is advised that patients are vaccinated against Neisseria meningitidis , Streptococcus pneumoniae , and if appropriate, Haemophilus influenzae type b according to local guidelines, at least 2 weeks prior to starting treatment, unless the risk of delaying treatment outweighs the risk of developing an infection. If immediate treatment is needed, administer the required vaccines as soon as possible; treat with prophylactic antibiotics until 2 weeks after vaccination. Revaccinate according to local guidelines if necessary. M
Side effects
Common or very common Arthralgia; diarrhoea; dizziness; gastrointestinal discomfort; headaches; increased risk of infection; nausea Uncommon Urticaria
Pregnancy
Use only if potential benefit outweighs risk (limited information available). M
