Drug action
Garadacimab is a fully human recombinant monoclonal antibody which binds to factor XIIa, thereby inhibiting the kallikrein cascade and the production of bradykinin, a potent vasodilator associated with inflammation and swelling in hereditary angioedema.
Indications and dose
**Prevention of recurrent attacks of hereditary angioedema (initiated under specialist supervision)**
- **Adult** (By Subcutaneous Injection): Loading dose 400 mg for 1 dose, then maintenance 200 mg every month, maintenance dose to be started 1 month after initial loading dose, consider discontinuing treatment if insufficient response after 3 months.

- **Child 12-17 years** (By subcutaneous injection): Loading dose 400 mg for 1 dose, then maintenance 200 mg every month, maintenance dose to be started 1 month after initial loading dose, consider discontinuing treatment if insufficient response after 3 months.
Contraindications
Not a treatment for acute attacks of angioedema (initiate individualised treatment for breakthrough attacks)
Side effects
Common or very common Abdominal pain; headache
Interactions
**Other interactions (3):**
- Garadacimab has only been studied as a monotherapy and not in combination with other products indicated for long-term prophylaxis of HAE.
- Based on population pharmacokinetic analyses, the use of analgesic, antibacterial, antihistamine, anti-inflammatory and anti-rheumatic medications had no effect on the PK of garadacimab.
- For breakthrough HAE attacks, use of rescue medications such as plasma-derived and recombinant C1-INH or icatibant had no effect on the PK of garadacimab.
Pregnancy
Preferable to avoid (limited or no information available; animal studies do not indicate toxicity). M
Breast feeding
Use only if potential benefit outweighs risk (possible risk from transfer of antibodies to infant during first few days after birth). M
