Drug action
Evinacumab is a recombinant human monoclonal antibody that binds to and blocks the angiopoeitin-like 3 protein (ANGPTL3) to prevent its inhibition of lipoprotein and endothelial lipase, thereby lowering triglycerides and cholesterol.
Indications and dose
**Homozygous familial hypercholesterolaemia (under expert supervision)**
- **Adult** (By Intravenous Infusion): 15 mg/kg every 4 weeks.
**Homozygous familial hypercholesterolaemia (under expert supervision) for evinacumab**
- **Child 6 months-17 years** (By intravenous infusion): 15 mg/kg every 4 weeks.
Side effects
Common or very common Abdominal pain; asthenia; constipation; dizziness; increased risk of infection; influenza like illness; infusion related reaction; nausea; pain; rhinorrhoea Uncommon Hypersensitivity
Interactions

- No interacting mechanisms between evinacumab and other lipid-lowering medications have been observed.
Pregnancy
Avoid unless potential benefit outweighs risk (toxicity in animal studies). M
Breast feeding
Specialist sources indicate use with caution, especially if breast-feeding a neonate or pre-term infant (no information available). Large molecular weight suggests limited excretion into milk and drug molecule likely to be partially destroyed in the infant's gastro-intestinal tract.
