Clinical overview
Diazepam is one of the most widely used benzodiazepines in clinical practice and remains a cornerstone drug in emergency medicine and neurology. It is the drug of choice for status epilepticus (IV diazepam is first-line in the emergency setting, per Indian and international guidelines), acute anxiety states, alcohol withdrawal syndrome, and skeletal muscle spasm. Its rapid onset of action after IV administration (1-3 minutes) makes it invaluable in seizure emergencies. Diazepam is highly lipophilic, which explains its rapid CNS penetration but also its extensive redistribution to adipose tissue, leading to a prolonged elimination half-life of 20-100 hours. The active metabolite desmethyldiazepam (nordiazepam) further extends its duration of action. In Indian clinical practice, diazepam is widely available and affordable across government and private hospitals. It is listed in the National List of Essential Medicines (NLEM) 2022. The drug undergoes hepatic metabolism via CYP3A4 and CYP2C19 and should be dose-reduced in elderly patients and those with hepatic impairment, who are at greater risk of oversedation and falls. Flumazenil is the specific benzodiazepine antagonist used for reversal in overdose situations.
Pharmacological class
Diazepam belongs to the Benzodiazepine (anxiolytic, anticonvulsant, skeletal muscle relaxant, sedative-hypnotic) class. Diazepam binds to the GABA-A receptor at the benzodiazepine binding site, enhancing the frequency of chloride ion channel opening in response to GABA. This potentiates inhibitory neurotransmission throughout the CNS. Unlike barbiturates, benzodiazepines do not directly open the chloride channel in the absence of GABA, which accounts for their wider safety margin.

Regulatory status
Diazepam is classified under Schedule H in India under the Drugs and Cosmetics Act, 1940. Listed under Schedule H of the Drugs and Cosmetics Rules; also controlled under the NDPS Act when used for de-addiction. Prescription-only; must bear the Schedule H warning label.

Dosing
Status epilepticus: 5-10 mg IV at 2-5 mg/min, may repeat once after 10-15 minutes (max 30 mg). Anxiety: 2-10 mg PO 2-4 times daily. Muscle spasm: 2-10 mg PO TDS-QDS. Elderly: start at half the adult dose. Children (febrile seizures): 0.2-0.5 mg/kg rectally. Avoid IM route -- erratic absorption from deltoid; if IM is necessary, use the gluteal site.
Contraindications
- Acute narrow-angle glaucoma
- Severe respiratory depression
- Sleep apnoea syndrome
- Myasthenia gravis (can worsen weakness)
- Severe hepatic insufficiency
- Known hypersensitivity to benzodiazepines
Adverse effects
- Sedation and drowsiness (most common)
- Ataxia and impaired coordination
- Anterograde amnesia
- Paradoxical excitement (especially in elderly and children)
- Respiratory depression (particularly with IV use)
- Dependence and withdrawal seizures with chronic use
- Neonatal floppy infant syndrome (if used near term)
- Thrombophlebitis at injection site
Drug interactions
- Alcohol and other CNS depressants -- additive sedation and respiratory depression
- CYP3A4 inhibitors (ketoconazole, erythromycin) -- increased diazepam levels
- Opioids -- heightened risk of fatal respiratory depression (FDA boxed warning)
- Phenytoin -- diazepam may alter phenytoin levels unpredictably
- Cimetidine -- inhibits diazepam metabolism, increasing half-life
Pregnancy and lactation
Category D. Crosses the placenta freely. First trimester use associated with increased risk of cleft lip/palate (though absolute risk is low). Use near term causes neonatal floppy infant syndrome, hypothermia, and respiratory depression. Avoid in pregnancy unless benefit clearly outweighs risk (e.g., life-threatening status epilepticus).

A 28-year-old man is brought to the emergency department with continuous tonic-clonic seizures lasting 15 minutes. His wife reports he has been non-compliant with his antiepileptic medications. What is the most appropriate initial management? Answer: IV diazepam (5-10 mg at 2-5 mg/min) is the first-line drug for status epilepticus. Lorazepam IV is an alternative first-line agent. Why is IV diazepam preferred over IM diazepam?
IM diazepam has erratic and slow absorption due to the drug's high lipophilicity and precipitation at the injection site, particularly in the deltoid muscle. IV administration provides rapid and predictable CNS levels within 1-3 minutes, which is critical in status epilepticus.
What is the difference between diazepam and lorazepam in status epilepticus?
