Atenolol is a cardioselective beta-1 blocker used primarily for rate control in atrial fibrillation, chronic stable angina, and secondary prevention after myocardial infarction.

Clinical overview
Atenolol is a hydrophilic, cardioselective beta-1 blocker that was once the most prescribed antihypertensive worldwide. While the LIFE trial and subsequent meta-analyses have downgraded beta-blockers from first-line hypertension therapy (they are no longer first-line in most guidelines including Indian Hypertension Guidelines 2019 for uncomplicated hypertension), atenolol remains critically important in rate control for atrial fibrillation, secondary prevention post-myocardial infarction, and chronic stable angina. Its hydrophilicity means it does not cross the blood-brain barrier readily, resulting in fewer CNS side effects (insomnia, nightmares) compared to lipophilic beta-blockers like propranolol and metoprolol. Atenolol is renally excreted and requires dose reduction in renal impairment, unlike metoprolol which is hepatically metabolised. In Indian practice, atenolol remains extremely popular due to its low cost, once-daily dosing, and wide familiarity among practitioners. It is a staple of Jan Aushadhi centres and primary health centres across the country. Fixed-dose combinations with amlodipine and chlorthalidone are widely prescribed. Students must understand that while atenolol reduces heart rate and angina symptoms effectively, it has not shown mortality benefit in uncomplicated hypertension -- a point frequently tested in NEET PG.
Pharmacological class
Atenolol belongs to the Beta-Adrenergic Blockers (Cardioselective, Beta-1 Selective) class. Selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, myocardial contractility, and renin release. At therapeutic doses, it has minimal effect on beta-2 receptors in bronchial and vascular smooth muscle, though selectivity is lost at higher doses.
Indian brand names and formulations
Available as: Aten (Zydus), Tenormin (AstraZeneca/Abbott), Betacard (Torrent), Atelol (IPCA).
Tablets: 25 mg, 50 mg, 100 mg. Fixed-dose combinations available with amlodipine, chlorthalidone, nifedipine, and hydrochlorothiazide.
Regulatory status
Atenolol is classified under Schedule H in India under the Drugs and Cosmetics Act, 1940. Prescription-only. One of the most affordable cardiovascular drugs on the National List of Essential Medicines (NLEM).
Indications
- Hypertension (no longer first-line for uncomplicated hypertension per current guidelines)
- Chronic stable angina
- Rate control in atrial fibrillation and atrial flutter
- Secondary prevention post-myocardial infarction
- Supraventricular tachycardia
Dosing
Adults: 25-50 mg once daily, may increase to 100 mg. Post-MI: 50-100 mg daily. Renal impairment (CrCl 15-35 mL/min): maximum 50 mg daily. CrCl <15 mL/min: maximum 25 mg daily or 50 mg on alternate days. Haemodialysis: give dose after dialysis.
Contraindications
- Severe bradycardia (HR <45 bpm)
- Second- or third-degree AV block (without pacemaker)
- Decompensated heart failure
- Cardiogenic shock
- Phaeochromocytoma (without prior alpha-blockade)
- Sick sinus syndrome (without pacemaker)
Adverse effects
- Bradycardia (dose-dependent)
- Cold extremities (reduced peripheral perfusion)
- Fatigue and exercise intolerance
- Erectile dysfunction (particularly troublesome in younger patients)
- Masked hypoglycaemia symptoms in diabetics (tachycardia blunted, but sweating preserved)
- Rebound hypertension and tachycardia if abruptly withdrawn
Drug interactions
- Verapamil or diltiazem: additive negative chronotropic and dromotropic effects; risk of severe bradycardia and AV block
- Clonidine: if both used, withdraw beta-blocker FIRST, then clonidine, to avoid rebound hypertensive crisis
- NSAIDs: may attenuate antihypertensive effect by sodium and water retention
- Insulin and sulphonylureas: may mask tachycardia of hypoglycaemia
Pregnancy and lactation
Category D. Associated with intrauterine growth restriction (IUGR) and neonatal bradycardia. Labetalol is the preferred beta-blocker in pregnancy.
Exam-style clinical scenario
A 60-year-old man presents with atrial fibrillation and a ventricular rate of 130 bpm. He has COPD but no acute bronchospasm. Which beta-blocker is appropriate for rate control? Atenolol -- being beta-1 selective, it can be used cautiously in stable COPD for rate control, though selectivity is dose-dependent.

Rs 10-30 for a strip of 14 tablets (50 mg). Among the cheapest cardiovascular drugs available.

Frequently Asked Questions
Why is atenolol no longer first-line for hypertension?
The LIFE trial (2002) showed losartan was superior to atenolol in reducing cardiovascular morbidity and mortality in hypertensive patients with LVH. Subsequent meta-analyses confirmed that beta-blockers do not reduce stroke or all-cause mortality compared to other antihypertensives in uncomplicated hypertension.
Can atenolol be used in asthma?
Atenolol is relatively contraindicated in asthma. While it is beta-1 selective, selectivity is dose-dependent and lost at higher doses. It may precipitate bronchospasm. If a beta-blocker is absolutely necessary in a patient with reactive airway disease, use with extreme caution at the lowest dose.
How does atenolol differ from metoprolol for NEET PG?
Atenolol is hydrophilic (fewer CNS effects, renal excretion, needs dose adjustment in CKD), while metoprolol is lipophilic (more CNS penetration, hepatic metabolism via CYP2D6, no renal dose adjustment). Metoprolol succinate (not tartrate) has mortality benefit in heart failure (MERIT-HF trial).
