Why ECG interpretation matters for exams
ECG questions appear in every major Indian postgraduate medical entrance. NEET-PG, INI-CET and FMGE each allocate marks to rhythm recognition, ACS pattern identification and conduction defect classification. Getting these marks requires a repeatable method, not pattern-matching from memory.

The five-step systematic approach
Every strip in the collection is annotated using the same five-step framework. Learn it once, apply it to any 12-lead ECG in the exam or on the ward.
Rate
Calculate ventricular rate using the 300-method or 6-second strip count. Identify bradycardia (<60) and tachycardia (>100).
Rhythm
Assess regularity, identify P waves, determine P-QRS relationship and classify sinus versus non-sinus rhythms.
Axis
Use leads I and aVF to determine the frontal plane axis. Recognise left and right axis deviation and their clinical significance.
Intervals
Measure PR interval, QRS duration and QTc. Identify prolongation patterns that point to specific conduction defects or drug effects.
Morphology
Analyse ST segments, T waves, Q waves and R-wave progression. Recognise STEMI criteria, strain patterns and bundle branch block morphology.

How to use this collection for exam preparation
Start with the basic-grade strips to establish your systematic approach. Work through each tracing using the five-step method before checking the annotation. The goal is to build a reliable reading habit, not to guess the diagnosis from the title.
Move to intermediate strips once you can consistently identify rate, rhythm and axis within 30 seconds. These introduce overlapping findings such as atrial fibrillation with ST changes or bundle branch block with axis deviation, which is where exam questions draw their difficulty.
The advanced strips present the patterns that distinguish high scorers: subtle posterior MI from reciprocal changes, differentiating SVT from VT using Brugada criteria, and recognising hyperkalaemia-related ECG progression. These appear as single best answer questions in NEET-PG and as image-based stems in INI-CET.
Frequently asked questions
Do I need prior ECG knowledge to start?
No. The basic-grade strips assume you know the anatomy of a 12-lead ECG but not the interpretation. The five-step method is explained with each strip, so you learn the framework as you practise.
How are difficulty levels assigned?
Basic strips show one clear finding (e.g. sinus bradycardia, obvious STEMI). Intermediate strips combine two findings or require interval measurement. Advanced strips present subtle or overlapping patterns that require systematic exclusion.
Are these mapped to specific NEET-PG or INI-CET topics?
