Two Exams, One Syllabus, Different Approaches
INI-CET and NEET PG are the two major postgraduate medical entrance exams in India. While they test the same MBBS curriculum, they differ significantly in question design, difficulty and what they reward. Understanding these differences is essential for candidates appearing for both.
Pattern Comparison
NEET PG: 200 MCQs, 3 hours 30 minutes, conducted by NBEMS, negative marking of 1 mark per wrong answer (4 marks per correct). INI-CET: 200 MCQs, 3 hours, conducted by AIIMS New Delhi, negative marking of 1/3 mark per wrong answer (1 mark per correct).
The time pressure is slightly higher in INI-CET (200 questions in 180 minutes vs 200 questions in 210 minutes for NEET PG), but the difference is marginal.
Question Style Differences
NEET PG questions have evolved to include more clinical vignettes, but still include a significant proportion of direct factual recall questions. A typical NEET PG question might present a clinical scenario and ask for the diagnosis, drug of choice, or investigation of choice.
INI-CET questions tend to be more concept-driven and mechanistic. A typical INI-CET question might present a clinical image (histopathology slide, X-ray, ECG) and ask for interpretation, or present a biochemical pathway and ask about the consequence of an enzyme deficiency. Image-based questions are more frequent in INI-CET.
Difficulty and Competition
INI-CET has fewer seats (approximately 2,500-3,000 per session across all INIs) compared to NEET PG (approximately 45,000+ seats across India). However, the candidate pool for INI-CET is also smaller, as many candidates do not apply for INI-CET if they do not aspire to INI seats.
The quality of questions in INI-CET is generally considered higher. Questions are often set by faculty from AIIMS and other premier institutes, and they tend to test understanding rather than memorisation.
Preparing for Both Simultaneously
The most efficient approach is building a strong conceptual foundation first (this serves both exams), then supplementing with exam-specific practice. For NEET PG, practice clinical vignette-based MCQs, drug-of-choice questions, and time management for the 3.5-hour format. For INI-CET, practice image-based questions, mechanism-focused questions, and develop comfort with the slightly faster pace.
Subject-wise, the priorities are similar: Medicine, Surgery, Pharmacology and Pathology carry the highest combined weightage in both exams. The difference is in how these subjects are tested -- NEET PG leans toward clinical application while INI-CET leans toward mechanistic understanding.

Is the negative marking different?
NEET PG deducts 1 mark for each incorrect answer (out of 4 marks per correct answer, so 25% penalty). INI-CET typically deducts 1/3 mark for each incorrect answer (out of 1 mark per correct answer, so 33% penalty). This makes INI-CET slightly more punishing for wrong answers proportionally.
