FMGE Exam Structure
The Foreign Medical Graduates Examination (FMGE) is structured as a 300-question, computer-based test (CBT) conducted by NBEMS. The exam is divided into two papers, each with 150 questions and a time limit of 2 hours 30 minutes per paper.
Paper 1: Pre-Clinical and Para-Clinical Subjects

High-yield topics in Paper 1 include neuroanatomy and embryology (Anatomy), renal and cardiac physiology (Physiology), metabolic pathways and enzyme deficiencies (Biochemistry), drug of choice and adverse effects (Pharmacology), haematological and systemic pathology (Pathology), bacteriology and virology (Microbiology), and medicolegal sections of the IPC (Forensic Medicine).
Paper 2: Clinical Subjects
Paper 2 covers Medicine (approximately 25-30 questions), Surgery (approximately 20-25 questions), Obstetrics and Gynaecology (approximately 15-20 questions), Paediatrics (approximately 10-15 questions), Ophthalmology (approximately 10-12 questions), ENT (approximately 8-10 questions), Community Medicine and PSM (approximately 15-20 questions), Orthopaedics (approximately 8-10 questions), Dermatology (approximately 5-8 questions), Psychiatry (approximately 5-8 questions), Anaesthesiology (approximately 5-8 questions), and Radiology (approximately 5-8 questions).
High-yield topics in Paper 2 include cardiology, endocrinology and infectious diseases (Medicine), GI surgery and trauma management (Surgery), high-risk pregnancy and contraception (OBG), neonatology and immunisation (Paediatrics), glaucoma and retinal disorders (Ophthalmology), and national health programmes and biostatistics (Community Medicine).
Scoring and Passing Criteria
Each correct answer earns 1 mark. There is no negative marking -- incorrect and unanswered questions both score zero. The total score is out of 300, and the passing mark is 150 (50%). This means candidates should attempt every question, even when unsure, since there is no penalty for guessing.
The pass rate for FMGE has historically been variable, ranging from approximately 10% to 25% in different sessions. The relatively low pass rate underscores the importance of systematic preparation and comprehensive coverage of the MBBS syllabus.
Subject-Wise Preparation Strategy
Given the pass/fail nature of FMGE, the strategy should maximise the total score rather than aiming for perfection in any single subject. A recommended approach involves strong command of the "Big Four" (Medicine, Surgery, Pharmacology, Pathology) which together account for nearly 100 questions, solid coverage of Anatomy, Physiology, Community Medicine and OBG for another 80-90 questions, and adequate preparation of remaining subjects for the final 110-120 questions.
Since there is no negative marking, time management is about coverage rather than caution. Candidates should aim to attempt all 300 questions, spending more time on clinical questions that require reasoning and less on factual recall questions.
